See below for the link to the request form for Central Registry Clearance. This form is required in Michigan in order for adults to visit Summer Camps. It is best if you fill it out now and email it in for review and approval. We will need a copy of your approval when it is returned. Thank you for your participation in this effort.
Camp Organizations and Children’s Campsites
Camp organizations and children’s campsites requesting a Central Registry clearance on employees or volunteers who are 21 years of age or older must complete the BCHS-Camps 001, Request for Central Registry Clearance form and submit it to the Bureau of Licensing and Regulatory Affairs (LARA).
